If you don’t have insurance or choose not to use insurance, you can get a Good Faith Estimate (GFE) in writing of the expected charges for items and services before you get care. You can ask for a GFE any time, even if you haven’t scheduled yet. This page explains who qualifies, when you’ll get it, what it includes, and how to dispute a bill.

Who qualifies

  • Uninsured patients.

  • Self-pay patients (you have insurance but choose not to use it for a specific visit or service).
    (If you plan to use insurance for your visit, the GFE rules below do not apply.)

When you’ll receive your GFE

  • If you schedule care 3–9 business days in advance → we send your GFE within 1 business day of scheduling.

  • If you schedule care 10+ business days in advance → we send your GFE within 3 business days of scheduling.

  • If you request a GFE before scheduling → we send your GFE within 3 business days of your request.

What your GFE includes

  • The expected charges for items and services we reasonably anticipate providing for your visit or treatment plan.

  • When reasonably expected, estimates from other providers or facilities involved in your care.

  • For recurring services (e.g., ongoing therapy or HRT management), one GFE may cover up to 12 months of care; we’ll issue an updated estimate if your plan changes.

If your bill is higher than your estimate

If any provider/facility bills you $400 or more above the total expected charges listed for that provider/facility on your GFE, you can start a dispute (Patient-Provider Dispute Resolution) with HHS within 120 days of the first bill. A $25 administrative fee applies. Start at cms.gov/nosurprises or call 1-800-985-3059.

How to request a GFE (or a copy)

Email billing@identityalaska.org, call 907-865-3247, or ask at check-in. Tell us your name, DOB, service you’re seeking, preferred format (email/mail/portal), and preferred language. We keep GFEs in your record for 6 years; you can request prior GFEs at any time.

Important notes

  • A GFE is not a bill and not a contract. Actual charges may change if your plan of care changes or if you need different services; we will issue an updated estimate when that happens.

  • Language assistance and auxiliary aids (qualified interpreters; large print, braille, audio, accessible electronic formats) are free—ask any staff member or contact our Section 1557 Coordinator.

  • For emergencies or urgent care, we will provide information as soon as practicable.
