Identity Closing Health Clinic and Transitioning Patient Care

It is with a heavy heart that we announce the closing of Identity Health Clinic. We recognize the anxiety this produces among our community, and we want to assure you that we will do everything in our power to provide a safe and stable transition for our patients.

This news comes with real loss. For many patients, Identity has been more than a clinic. Our community has built it into a place of trust, dignity, and care in a world where that is increasingly hard to find. We are deeply sorry for the disruption and hurt this closure causes, and our focus now is on helping patients move through this transition with as much care and clarity as possible.

The clinic has closed as of May 15, 2026, but our board can be reached via email to address the transition in the community.

It has truly been an honor to care for our community.

Thank you for helping us protect patients during this closure.

This community built Identity into a place of care, trust, and belonging. As we wind down services, we are working to uphold one of our core commitments: no patient should be sent to collections for medical bills because of this closure.
