Identity Closing Health Clinic and Transitioning Patient Care
It is with a heavy heart that we announce the closing of Identity Health Clinic. We recognize the anxiety this produces among our community, and we want to assure you that we will do everything in our power to provide a safe and stable transition for our patients.
This news comes with real loss. For many patients, Identity has been more than a clinic. Our community has built it into a place of trust, dignity, and care in a world where that is increasingly hard to find. We are deeply sorry for the disruption and hurt this closure causes, and our focus now is on helping patients move through this transition with as much care and clarity as possible.

Bridging Your Care to New Providers
If you are a current patient, please send us a portal message with your specific needs. This may include medication refill requests, referrals to other providers, medical records requests, or other important transition-related care needs.
Please be as specific as possible so we can help effectively. Because we are supporting many patients at once with a small team, please allow up to 4 business days for a response before calling to follow up.

Please see below for a downloadable Release of Information form to request medical records to complete and send to us directly by attaching the completed form to a message to your provider via your patient portal.
Please do not send ROI forms to us via email, and call if you need assistance with this process.
